Anger Management [Court Ordered – Criminal/Civil]

At Embrace, our Anger Management Program is between 4 to 8 sessions, depending on attorney direction or court orders.

    1. Call or Text Embrace’s Intake Specialist at 972.292.7092 EN/ES.
    2. Intake Specialist will accept a copy of your court-order or letter from attorney stating nature of legal case and anger management at YouMatter@EmbraceNewLife.com or faxed to 972.292.7092.
    3. We will tailor your Anger Management to the specific issues of your legal case.
    4. Each session will provide psychoeducation on the latest findings in self-regulation and managing anger.
    5. Good faith-estimate: $145 per session – complete program cost: $580 – $1,160.
    6. Letter verifying completion will only be provided at completion of last session.

Court-ordered Family Counseling

At Embrace, our Court-ordered Family Counseling program can be between 8 to 12 sessions, depending on presenting family issue.

    1. We do not accept cases where there is a history of domestic violence or where there are ongoing abuse criminal charges.
    2. Call or Text Embrace’s Intake Specialist at 972.292.7092 EN/ES.
    3. Intake Specialist will require and accept a copy of the most recent court-order specifying family counseling orders at YouMatter@EmbraceNewLife.com or fax to 972.292.7092.
    4. We will tailor your Family Counseling to the specific issues of your legal case. We require that BOTH biological parents consent to treatment.
    5. Each session will systemic family counseling based upon the Association of Family & Conciliatory Courts standards of court-informed family care.
    6. Good faith-estimate: $165 per session. This cost does not include any court-costs like affidavits, records to the court or fact testimony.
    7. Letter verifying completion will only be provided at completion of last session.

PTSD Treatment for Personal Injury

  • At Embrace, our PTSD services for Personal Injury are tailored to your specific needs.
    1. Call or Text Embrace’s Intake Specialist at 972.292.7092 EN/ES.
    2. Intake Specialist will require a ROI with your attorney or attorney’s document service that can be emailed to YouMatter@EmbraceNewLife or faxed to 972.292.7092.
    3. Intake Specialist will require your signature on our Legal Service Informed Consent that outline the differences in confidentiality of services when part of a legal filing.
    4. We will tailor your PTSD services to your specific case and document history of preexisting functioning, baseline and progress for legal record.
    5. Good faith-estimate: Starting at $165 per session.ย  One round of EMDR (8 sessions) is approximately $1,320.ย  Number of rounds depends on case complexity.
    6. Letter verifying baseline measures and progress are provided at attorney request. Each Letter/Affidavit is notarized and is $125 without medical records and $150 with medical records.

FMLA & Short-Term Disability

At Embrace, our FMLA & Short-Term Disability services are tailored to your need.

    • Call your HR department and inquire about your employee FMLA & Short-termed Disability benefits.
    • Call or Text Embrace’s Intake Specialist at 972.292.7092 EN/ES.
    • Intake Specialist will accept a copy of your FMLA forms as well as your Short-Term Disability certification form and all ROIs at YouMatter@EmbraceNewLife.com or fax 972.292.7092.
    • FMLA forms require a minimum of 2 sessions to initiate (intake). FMLA is recertified every two weeks and requires weekly sessions with your ENL therapist.ย  FMLA will not be certified after 1 visit.
    • Short-term Disability forms require a minimum of 2 sessions to initiate (intake). Short-term Disability is recertified every two weeks and requires weekly sessions with your ENL therapist.ย  Short-term Disability will not be certified for more than 2 weeks at a time to report progress to your insurance company.
    • Good faith-estimate: FMLA & Short-Term Disability Intake – $290 for 2 session intake, $290 for each additional sets of 2 weeks requested leave (or $145 per week and session on leave).
    • Letter verifying Return-to-Work will only be provided at completion of last session. If Return-to-Work requires physical clearance, this will need to be completed by physician.
