Requesting Your Health Care Records

Clients may request copies of their health care records directly from our practice.

How to request records:

  1. Submit a written request via email or in person.

  2. Include your full name, date of birth, and the specific records requested.

  3. Allow reasonable processing time in accordance with Texas law.
